I, too, encountered the broken .pdf link, but right-clicked and saved the form to my hard drive.  Once printed, it was blurry, but readable.  Not long after I sent in my check, I asked via DJML if there was a good email contact for status, and Mr Ringwald forwarded my query to the lady who is apparently in charge.  We exchanged a couple emails over the next few days and I got the info I needed, including that my badge would be mailed within the next few days.  This was some time ago and nothing has arrived.  Even if nothing ever arrives, I will have no problem plunking down my cancelled check upon my arrival.

I also left a phone message, which has not been returned.  Interestingly, the two other times I attended, each time my phone message was promptly returned.  Maybe this year there's different folk running the show?
