The Atlas number appears to be A 101 218

The tune on both sides is "Streakin" with composer listed as Frank LaVere.
It is "produced by Frank LaVere for Theodorop-Levy Prod.  The number in the
deadwax is #21 B 3 and I believe a very (VERY!) small 3. It also has TE
Leslie.

Any help would be appreciated, although it is just of interest -- not of
life threatening importance.
