I forgot to include the recording information.
As previously advised, the location was Chicago and the year was 1968. 
Additional research information is also included below.

•The  Art Hodes JAZZ ALLEY half-hour shows were directed by Robert Kaiser and broadcast by Chicago's educational television station WTTW
