D - depends on the drummer
A & B - both possible, depends on the tune
C - came to mind very often before I got married

It all depends on how well you know a song. It also differs per person, my wife Slidin Selena, usually picks the melody. She has absolute pitch so whatever chord structure, she hears it anyway.
I myself tend to use the chords more often, but when I know a tune very well, I think about nothing and just play.
Sometimes I think about what will be the next tune, as I usually choose them, and rarely make set lists. So I want a tune with different Key, tempo and mood.
