Just to confuse matters, bands I have worked with and still have their  
chord charts, charted the first 4 bars 3 different ways, as below:

1)  /  C - G+ / C - G+ / C - Am7 / D9 /

2)  /  C - G+ / C - Am / Em - Am / D7 /

3)  /  C - B9 / C - B9 /  C / D9 /

I like version #2.
