Everyone does not have the gift of self promotion or effectively selling 
themselves.  Many outstanding jazz players are so focused on jazz that all 
else goes away.  There are some other reasons too such as family 
obligations, the desire to have children, a home and a decent income. 
Truthfully some of the finest Jazzers I know are in military bands which 
does not allow them a lot of freedom to do other things or become well 
known.

Unfortunately the only way to produce a truly great musician seems to be 
poverty, dire necessity and few if any other prospects.  It seems to work 
that way in sports too.  You just don't seem to find many outstanding 
athletes or musicians coming from the well off or very wealthy classes. 
They are more likely to produce Doctors, Lawyers and CEO's.  The middle 
classes tend to produce artists that like security so while they may be very 
good they just don't hit the national scene.

When I had the chance to go do a lot of the things that would have been 
musically rewarding, there were things like kids, wife and car payments that 
seemed more important at the time.  I suspect that there are thousands of 
musicians like me out there.
