Jack Mitchell is spot-on. I can remember its use in Sydney in the 1950s.

The Oxford English Dictionary records its appearance in print as 
1967-Kings Cross Whisper (Sydney) xxxix. 9/1 (headline)    Musos blow 
cold ... Members of the Sydney symphony orchestra will work to rule.

Australians have a habit of ending abbreviations with an "o", hence

demonstration - demo
car registration - car rego
methylated spirits - metho
refugee - refo (now impolite)
