[Dixielandjazz] Salute to Basie concert

Dear friends.
I have a query I cannot answer.
Jack Teagarden appeared on a 'Salute to Basie concert' in late 1963 on which he
sang "Basin Street Blues" and "Lover".
The concert has apparently been released on LP and is supposed to be the last
recorded stuff by JT just prior to his unfortunate death in Jan 1964.
Queries:
What was the date of the concert?
Where was it held?
On what LP (CD?) was it issued and under whose name (if any)?
Who was in the band backing Tea's vcls?
