You might try using a color photocopier in the reduction mode. Then trim to
size. 

Another possibility is to mount a digital camera on a copystand or tripod
and shoot a photo of the cover. Then be print it on one of those pre-scored
blanks.

Both of these options would be somewhat expensive. Let us know what you
settle on.
